Calidad muscular, adiposidad abdominal y rendimiento funcional en mujeres mayores con obesidad definida por DEXA

Resumen

Introducción: el envejecimiento femenino se asocia a redistribución adiposa, deterioro muscular y disminución de funcionalidad. La integración de la evaluación de composición corporal y funcionalidad podría mejorar la caracterización clínica de la obesidad.

Objetivo: analizar la relación entre calidad muscular, adiposidad abdominal, parámetros hemodinámicos y rendimiento funcional en mujeres mayores con obesidad.

Metodología: estudio observacional, transversal y analítico en 131 mujeres mayores comunitarias. La composición corporal se evaluó mediante absorciometría dual de rayos X (DEXA), obteniéndose porcentaje de grasa, masa magra apendicular, tejido adiposo visceral y subcutáneo. La calidad muscular se estimó mediante el índice de calidad muscular. El rendimiento funcional se evaluó mediante velocidad de marcha, Timed Up and Go, prueba de levantarse y sentarse cinco veces, equilibrio unipodal y prueba Step 2 minutos. Se aplicaron análisis de varianza, pruebas robustas de Welch y regresión lineal múltiple.

Resultados: 23.7% presentó deterioro de la calidad muscular y el 54.2% obesidad clínica. Las participantes con menor calidad muscular tuvieron peor equilibrio, movilidad funcional y resistencia. El tejido adiposo visceral se asoció con presión arterial diastólica, mientras que la edad se relacionó con mayor presión arterial sistólica y peor rendimiento funcional. Además, el índice entre tejido adiposo visceral y subcutáneo aumentó progresivamente con la edad.

Discusión: los resultados coinciden con estudios que describen asociaciones entre deterioro muscular, redistribución adiposa abdominal y disminución funcional.

Conclusiones: menor calidad muscular se asoció con peor funcionalidad física en mujeres mayores con obesidad. Además, el envejecimiento avanzado se relacionó con redistribución adiposa abdominal y deterioro progresivo del rendimiento funcional.
