Qualidade muscular, adiposidade abdominal e desempenho funcional em mulheres idosas com obesidade definida por DEXA

Introdução: o envelhecimento feminino está associado à redistribuição do tecido adiposo, deterioração muscular e diminuição da funcionalidade. A integração da composição corporal e da funcionalidade pode melhorar a caracterização clínica da obesidade.
Objectivo: analisar a relação entre a qualidade muscular, a adiposidade abdominal, os parâmetros hemodinâmicos e o desempenho funcional em mulheres idosas com obesidade.
Metodologia: foi realizado um estudo observacional, transversal e analítico com 131 mulheres idosas residentes na comunidade. A composição corporal foi avaliada através de absorciometria de raios X de dupla energia (DXA), obtendo-se a percentagem de gordura corporal, a massa magra apendicular, o tecido adiposo visceral e o tecido adiposo subcutâneo. A qualidade muscular foi estimada através do índice de qualidade muscular. O desempenho funcional foi avaliado através da velocidade da marcha, do teste Timed Up and Go, do teste de sentar e levantar cinco vezes, do equilíbrio unipodal e do teste de dois minutos de degrau. Foram realizadas análises de variância, testes robustos de Welch e regressão linear múltipla.
Resultados: 23,7% apresentavam qualidade muscular comprometida e 54,2% obesidade clínica. Os participantes com menor qualidade muscular apresentaram pior equilíbrio, mobilidade funcional e resistência. O tecido adiposo visceral foi associado à pressão arterial diastólica, enquanto a idade esteve relacionada com uma pressão arterial sistólica mais elevada e um pior desempenho funcional. Além disso, a proporção entre o tecido adiposo visceral e subcutâneo aumentou progressivamente com a idade.
Discussão: os achados são consistentes com estudos que descrevem associações entre deterioração muscular, redistribuição da gordura abdominal e declínio funcional.
Conclusões: menor qualidade muscular esteve associada a pior funcionalidade física em mulheres idosas com obesidade. Além disso, o envelhecimento avançado esteve relacionado com a redistribuição da gordura abdominal e com a deterioração progressiva do desempenho funcional.
