Impacto de un programa de ejercicio físico mediante telerehabilitación cardíaca en pacientes con insuficiencia cardíaca: un ensayo clínico aleatorizado y controlado

Introducción: La rehabilitación cardíaca se destaca como la principal recomendación para pacientes con insuficiencia cardíaca. La falta de adherencia, atribuida a la falta de autocuidado, educación y seguimiento, impacta negativamente la enfermedad. La telerehabilitación se presenta como una herramienta valiosa para lograr mejoras similares a las de la rehabilitación cardíaca en la capacidad aeróbica funcional, la depresión/ansiedad y la calidad de vida. El objetivo principal es determinar los efectos de un programa de ejercicio físico y Seguimiento educativo telefónico, mediado por Telerehabilitación Cardíaca (STRC), en pacientes con insuficiencia cardíaca sobre la capacidad aeróbica funcional, en comparación con la Telerehabilitación Cardíaca (TRC) y la Rehabilitación Cardíaca ambulatoria Convencional (RCC).
Método: Ensayo clínico controlado aleatorizado de tres brazos (RCC, TRC, STRC) siguiendo las recomendaciones CONSORT para ensayos clínicos. Se evaluaron la capacidad aeróbica funcional, variables clínicas, ansiedad/depresión y calidad de vida relacionada con la salud.
Resultados: La capacidad aeróbica funcional en todos los grupos presentó una mejora estadísticamente significativa en la distancia recorrida en la prueba de marcha de 6 minutos (p < 0,05) al inicio y al final de la intervención, con una diferencia media de 73,36 ± 2,56, superior a la observada en los grupos TRC y RCC.
Conclusiones: Se observan mejoras más favorables con la implementación de la telerehabilitación cardíaca, la educación y el seguimiento telefónico en la capacidad aeróbica funcional.
